CEU Sheet (pdf file)

Enter your name.

Enter the name of the Course, in Course Title.

Your License number is entered next to the license type located in the upper right corner of the form.

Signature of applicant is where you sign and date.

We will fill in the Course #, and return a copy to you.

SIGN IN Sheet (pdf file)

Print your name, address, phone number, and sign your name.

Each Course is 3 hours CEU for Az License holders.

Each Course is $25.00

Then send BOTH Forms with your check to:
